LIABILITY WAIVER, ASSUMPTION OF RISK & MEDIA RELEASE

LIABILITY WAIVER & ASSUMPTION OF RISK

In consideration for participation in any Champions United Track & Field Series meet, competition, or related event activity (collectively referred to as the “Event”), I, on behalf of myself and/or the minor participant listed above, acknowledge and agree to the following:

1. Acknowledgment of Inherent Risks

I understand that participation in track & field activities involves inherent risks, including but not limited to running-related injuries, falls and collisions, muscle strains and sprains, heat exhaustion and dehydration, equipment-related injuries, and the possibility of serious bodily injury, permanent disability, paralysis, or death. I voluntarily assume all risks associated with participation in the Event, whether known or unknown.

2. Physical Condition & Medical Eligibility

I certify that the participant is physically fit, properly conditioned, and medically able to participate in track & field activities. I acknowledge that it is my responsibility to consult with a licensed physician regarding participation in strenuous athletic activities. I further affirm that any medical conditions, allergies, medications, or injuries that may affect safe participation have been disclosed to Champions United Track & Field Series.

3. Medical Treatment Authorization

In the event of illness, injury, or medical emergency, I authorize Champions United Track & Field Series staff, officials, volunteers, athletic trainers, and emergency medical personnel to obtain medical treatment deemed necessary for the participant. I understand and agree that I am solely responsible for any medical expenses incurred as a result of such treatment.

4. Release of Liability

To the fullest extent permitted by law, I hereby release, waive, discharge, and hold harmless Champions United Track & Field Series, LLC, its owners, directors, officers, employees, coaches, volunteers, officials, sponsors, venue operators, agents, contractors, and affiliated organizations (collectively “Released Parties”) from any and all claims, liabilities, demands, damages, costs, expenses, or causes of action arising out of or related to participation in the Event, including claims arising from the negligence of the Released Parties. This release includes, but is not limited to, personal injury, illness, property damage, economic loss, and wrongful death.

5. Compliance With Rules & Conduct

I agree that the participant will follow all Event rules, facility policies, safety instructions, and directions provided by officials, coaches, and staff. I understand that failure to comply with rules or engaging in unsafe, disruptive, abusive, or unsportsmanlike conduct may result in removal from the Event without refund.

6. Weather, Event Changes & Refund Policy

I understand that outdoor sporting events may be affected by weather, facility conditions, scheduling conflicts, or unforeseen circumstances. Champions United Track & Field Series reserves the right to modify, postpone, reschedule, or cancel events at its discretion for safety or operational reasons. I understand that registration fees may be non-refundable except as determined by Champions United Track & Field Series policy.

7. Communicable Disease Acknowledgment

I understand that participation in athletic events may involve exposure to communicable illnesses, including but not limited to viruses, bacteria, and infectious diseases. I voluntarily assume all risks related to exposure and release the Released Parties from claims relating to illness or disease exposure associated with participation.

8. Indemnification

I agree to indemnify, defend, and hold harmless the Released Parties from any claims, damages, losses, liabilities, or expenses arising out of my actions or omissions, the participant’s conduct, violation of Event rules, or any claims brought on behalf of the participant.

9. Venue & Property Damage Responsibility

I understand that participants and spectators are expected to respect all facilities and venue property. I accept financial responsibility for any damage caused by the participant due to intentional misconduct or reckless behavior.

10. Electronic Signature Consent

I agree that electronic acceptance of this waiver and participation agreement shall have the same legal force and effect as a handwritten signature.

11. Governing Law

This Agreement shall be governed by and interpreted in accordance with the laws of the State of Georgia.

MEDIA, PHOTO & VIDEO RELEASE

I hereby grant Champions United Track & Field Series, LLC permission to photograph, record, livestream, and use the participant’s name, image, likeness, voice, performance, and appearance in any photographs, videos, digital recordings, or promotional materials created during participation in Events.

I understand and agree that such materials may be used for websites, social media, marketing campaigns, advertisements, promotional videos, livestream broadcasts, print materials, and news or media coverage.

I acknowledge and agree that no compensation will be provided for use of these materials, Champions United Track & Field Series retains ownership rights to all media content, I waive any right to inspect or approve final media products, and I release Champions United Track & Field Series from any claims related to the use of such media.

I HAVE READ THIS AGREEMENT CAREFULLY AND FULLY UNDERSTAND ITS TERMS.
